Patient and Family/Caregiver Instruction :

Instructs patients, families and caregivers in exercise regime and the use and care of therapeutic appliances

Performs physical therapy procedures in the patient’s home.

Administers non-complex therapeutic exercises, therapeutic massage, traction, heat, light, cold, water and electrical modalities to patients with relatively stable conditions.

Instructs, motivates, and assists patients in learning and improving functional activities.

Teaches basic principles of body mechanics to patients and families.

Instructs patients and family members on the proper use of prosthetic devices and in the use and care of therapeutic appliances.

Instructs patients, family members and other team members in the patient’s treatment regime, as indicated.

Communication :

Clearly communicates patient’s progress and response to treatment to the Physical Therapist

Communicates changes in the patient’s condition to the Physical Therapist

Confers with the Physical Therapist, attending physician, home health team members and patient and/or family/caregivers to promote coordinated and comprehensive care

Communicates the Plan of Care to other members of the home health team and patient/family and/or caregivers all under direction of the PT.

Appropriately calls in emergencies to supervisor or agency

Calls complaints/concerns in to agency and explains complaint process to family.

Utilizes safety tools and reinforces teaching with patient/family and documents the same.

Documentation :

Accurately completes modifications to the Plan of Care, as authorized by the physician, after subsequent authorization, instruction and supervision by the Physical Therapist.

Completes Clinical and Progress Notes in accordance with Ascendant Home Health Care policies, state and federal guidelines and regulations.

Ensures all patient paperwork is accurate, complete and written at time of service provision or immediately thereafter and turned into agency within 24 hours of service/visit completion.
