Clinical Manager

1. Makes patient and personnel assignments: Facilitates schedules for patient visits by team members and informs team members of any special requirements for patient care.

2. Coordinates patient care with the interdisciplinary team and serves as a role model and resource person to team; assures the patient needs are continually assessed, with the development, implementation and updates of the individualized plan of care for each patient.

3. Conducts and participates in patient care conferences/team meetings using an interdisciplinary approach.

4. Facilitates discharge planning and coordinates with other disciplines to meet patient discharge needs.

5. Coordinates with the patient/family and significant others, physician responsible for home health plan of care, home health team members and other health care providers regarding patient care needs.

6. Ensures that documentation by each clinical team member is complete, accurate and timely per agency guidelines.

Personnel Management/Supervision

1. Reviews and provides feedback regarding completeness, accuracy and timeliness of clinical documentation.

2. Completes annual employee performance evaluations, assists in setting annual performance goals and provides feedback.

3. Oversees orientation of new clinical staff and completion of new employee’s skills checklist.

Operational Oversight

1. Coordinates patient caseloads to provide consistent quality care and visit frequency per agency established productivity guidelines.

2. Reviews schedules daily to ensure visit coverage and address specific patient/family issues.

3. Responsible for completion of weekend schedules and on-call rotation schedules.

4. Monitors, evaluates, and promotes collaborative, productive team functioning of all team members.

5. Directs staff to appropriate resources to enhance delivery of care.

6. Assists clinicians with clinical record documentation including computerized record.

7. Promotes open communication, positive physician/staff interactions and the timely resolution of issues, collaborating with others including the Administrator.

8. Assesses and adjusts staffing to provide for changing patient care needs.

9. Performs delegated responsibilities, utilizing the appropriate chain of command.

10. Assists Administrator and provides follow-up relative to events, complaints, concerns and/or issues in a timely manner

11. Collaborates in strategic planning with the Administrator obtaining input as needed from the home care team and staff supervised. .

12. In absence of Administrator, is capable of managing the agency and acting on his/her behalf.

13. Demonstrates professionalism through accountability and confidentiality.

14. Understands and follows confidentiality and HIPAA guidelines in order to implement the proper process for accessing/maintaining usage of electronic medical records.

15. Reports all appropriate/relevant information regarding the agency activities to the PAC.

3. Attends and organizes the PAC meetings.

4. Oriented to roles and responsibilities of Administrator in order to act on his/her behalf in his/her absence with the same responsibilities and obligations. Has completed competency assessment/skills checklist for Administrator role.
