History
Community Home Health Care began as a small organization comprised of highly trained nurses and therapists dedicated to improving the lives of individuals within the community. By providing excellent care, we grew into a well-recognized entity where people ask for us by name. Our promise is to continue the tradition of excellence by creating a happier, healthier community.

  1. Team Assessments – Each patient is evaluated by an RN accompanied by a physical or occupational therapist to accurately assess the patient's functional status

  3. Highest Scores – "Home Health Compare" rates Community Home Health Care HIGHEST in improving patient's ability in Ambulation, Transfers, Bathing, Getting in and out of Bed, etc.
  4. Multiple Modalities - Multiple Modalities are used in therapeutic evaluations/treatments. For instance, all therapists have access to Ultrasound, Electrical Stimulus, TEN's units, etc.
  5. Cognitive Screening and Care – Community provides a unique cognitive re-training program headed up by our Occupational Therapists.
  6. High Tech Care – Infusion Therapy for hydration, antibiotics, etc. is provided by our highly trained nursing staff. Therapists provide wound care including, but not limited to, the application of unna boots. Nursing staff are trained in the care of patients with physician ordered Wound Vac treatment. We work carefully with Infectious Disease Physicians, local wound care clinics, etc. to provide the highest level of care possible at home. Recently, we became one of the only agencies in the region to offer in-home IVIG Treatments.
  7. Updates - We stay updated on the very latest in the home health industry. One of our Board Members is a nationally recognized expert in home health care, providing us with the most up to date information possible on an ongoing basis.
  8. Follow Up - Phone visits are used between our routine visits to monitor patients' self care at home.
  9. Patient Satisfaction – Care evaluations at discharge indicate a very high degree of overall patient satisfaction; our scores are well above average.
