Application for a Certificate of Title (Form #VR-005) can be used for titling a vehicle. This form must be filled out with Asap Title Agent and Signed by Owners only or Agents with POA.

Affidavit Manufactured Home Converted to Real Property (Form #VR-451) is to be used when a manufactured home is being converted to real property.
Affidavit Manufactured Home Severed From Real Property (Form #VR-452) is to be used to certify under oath a manufactured home has been severed from real property. This is a walk in by the Title Agent. May need to return within specified business days to ASAP Tag & Title.
Application for Assigned Vehicle Identification Number (Form #VR-198) is used by customers who make homemade vehicles, certain kit vehicles, and for lost VINS. When this document is used applicant must then proceed to State Police for Inspection to Title. Then return to ASAP Tag & Title for normal registration process.
Application for a Corrected Title Due to a Name Change (Form #VR-448) is used to apply for a corrected title for a name change. ASAP Tag & Title process on behalf of applicant and the Title is mailed to the original owner to address on file.
Application for Duplicate Certificate of Title (Form #VR-018) can be used to apply for a duplicate title for a vehicle. When application is made; duplicate is mailed to the address on the original title unless proof of address change is verified by the owner of the vehicle at the time of application. ASAP Tag & Title can process with mailing address verified.
Application for Duplicate or Substitute Plates/Stickers (Form #VR-009) This form must be filled out and can be completed electronically with an Asap Title Agent if original is lost, stolen, damaged, or mutilated.
Application for Duplicate Security Interest Filing (Form #VR-048) is used to request a duplicate security interest filing. The completed form, along with the appropriate fees, can be faxed to the financing institution for completion of titling services where there is a recorded lien. Asap Title Agent will assist is the process but does not control response time of any institution.
Application for Historic or Street Rod Registration (Form #VR-096) is used to apply for historic or street rod license plates. You will need to fill out the form and provide all necessary documentation. ASAP Tag & Title has either tag for issuance along with the appropriate fees.
Application for Maryland County Pride Stickers (Form #VR-308) is used to apply for the Maryland County Pride Stickers. This form can be filled out and submitted wo Asap Title Agent along with the appropriate fee for processing. There is processing days for issuance.
Application for New Plates/Stickers & Transfer of Plates or Non-Title Trailers (Form #VR-008) can be used to apply for new plates and stickers or to transfer plates or non-title trailers. Completed with ASAP Title Agent.
Bill of Sale (Form #VR-181) is a form to establish the purchase price of your vehicle. Asap Title Agent will REQUIRE THIS FORM FOR ALL Title transfers for vehicles that are sold below FMV, Gifted, or 7 model years or less. Please have the seller to write a Bill of Sale if unable to print this form. NOTARY services are available on location of ASAP Tag & Title.
Certified Statement / Receipt (Form VR-461), is used in the event the registration plates are not available for return (e.g., lost, stolen), please complete VR-461 to explain exactly what occurred to the vehicle registration plates. Certification is required as Maryland Law requires customers to return their license plates to the MVA when the license plates are no longer needed. You may return to ASAP Tag & Title and obtain a receipt.
Change of Address Application (Form #VR-154), with which you can also apply to register to vote. This form can be filled out with an ASAP Title Agent can process with mailing address verified.
Off-Road Recreational Vehicle Permit Application (Form #VR-337 used by customers to apply for the Off Road Vehicle Titling Sticker that is placed on the rear of the off road vehicles. This sticker may be issued to current or new owners of Off Road Vehicles and is for vehicle and owner identification purposes only. This sticker does not allow the vehicle to be used on roadways or authorized DNR trails. This includes Motor Scooters, ATVs.
