One of the most time consuming aspects of working as a hand therapist, is splinting. As the splint complexity increases, so does the time investment and cost. To create a flexor/extensor tendon repair splint for proper rehab could take upwards of an hour, thus limiting direct patient care. Currently, the therapist may utilize multiple protocols and exercise regimes for optimal results of tendon repairs. This dynamic traction system can be used for flexor tendon repair following the Kleinert protocol, which can easily attach to any splint and can be quickly detached by the health care provider. The DiLoTrax device can also be utilized for radial, posterior interosseous nerve palsy, metacarpal arthroplasty, and extensor tendon repair all in a similar manner. The device is simply attached, aligned and adjusted to each individual patient.

Splint design is vital and the compliance of the patient is crucial for success. With proper splint design (that is also cosmetically pleasing) and efficient dynamic movement, the patient will be able to follow an exercise regime without any tangling or breaking of rubber bands/elastic strings and eliminating any catching onto clothing. This directly affects the outcome and maximizes function of the patient.

When using this device for flexor tendon repair following the Kleinert protocol it has a mid-palmer pulley with eight guides to ensure proper alignment. Increased flexion at the PIP and DIP joints have been found to produce greater tendon excursion and improved clinical results (May et al., 1992). Studies have shown that with all four fingers in dynamic flexion, patients regained better motion than historic controls. This resulted in improved active motion, regardless of the number of injured digits. Patients’ confidence and less fear of moving the injured digit, with other digits in synergy create less resistance in following the protocol. This results in less edema and joint stiffness.

This pre-fabricated unit can quickly and easily be used by the clinic, providing the therapist greater flexibility in treatment options.
