Personnel Management/Supervision

1. Supervises management staff including Director of Nursing and Business Manager in a manner that reflects patient care standards.

2. Oversees the staff interview process and selection of potential candidates for the home health team.

3. Oversees orientation of new staff and completion of new employee’s skills checklist (if applicable).

4. Completes annual employee performance evaluations, assists in setting annual performance goals and provides feedback.

Operational Oversight

1. Oversees the operation of the agency to see that patient caseloads are covered in order to provide consistent quality care and visit frequency per agency established productivity guidelines.

2. Guarantees that care is coordinated according to established guidelines and in compliance with Medicare Conditions of Participation.

3. Monitors, evaluates, and promotes collaborative, productive team functioning of all team members.

4. Promotes open communication, positive physician/staff interactions and the timely resolution of issues.

5. Assesses and adjusts staffing to provide for changing patient care needs.

6. Delegates responsibilities as appropriate, utilizing the chain of command.

7. Provides follow-up relative to events, complaints, concerns and/or issues in a timely manner.

Leadership

1. Demonstrates commitment to and support of the mission, vision, and values of the organization.

2. Manages competing demands for time and resources independently and prioritizes work responsibilities appropriately.

3. Plans, oversees, documents and effectively manages staff meetings.

4. Holds inservices for staff and sees that competencies are completed and all personnel files are maintained according to state and federal law in accordance with state guidelines and the Medicare Home Health Conditions of Participation.

5. Encourages collaboration among staff members to promote morale and productivity.

6. Effectively delegates responsibilities to appropriate members of management team.

7. Promotes effective planning and utilization of agency resources for efficient and cost- effective delivery of quality patient care.

8. Acts as a role model in the development of staff including training on appropriate documentation and processes.

9. Collaborates in strategic planning obtaining input as needed from the home care team and staff supervised.

10. Manages the agency according to State Licensure guidelines and Medicare Home Health Conditions of Participation.

11. Demonstrates professionalism through accountability and confidentiality.

12. Understands and follows confidentiality and HIPAA guidelines in order to implement the proper process for accessing/maintaining usage of electronic medical records and trains/oversees staff in the same.
