Coordinates effectively with other members of the Home Health Care Team :

In conjunction with patient, physician and home care team, develops the SLP plan of care based on patient needs and appropriate interventions.

Thoroughly and accurately screens, evaluates and documents the patient’s status and need for speech-language pathology services by the following methods :

Assessment for speech, language and/or cognitive-communication disorders involving body structure/function &/or activities/participation

Swallowing and feeding assessment

Speech sound assessment to evaluate for strengths and weaknesses in speech sound discrimination and production

Spoken and written language assessment

Severe communication impairment assessment

Prosthetic/adaptive device assessment

Fluency assessment

Motor speech assessment

Voice assessment

Resonance and nasal airflow assessment

Orofacial myofunctional assessment

Cognitive-communication and/or language abilities associated with auditory processing disorders (APD) assessment

Aural rehabilitation assessment

Other relevant/appropriate assessments

Develops the plan of care based on patient specific assessment and reflects appropriate speech-language pathology interventions based on the American Speech-Language Hearing Association Guidelines which include :

Patient-specific plan of care with interventions tied back to the assessed patient needs for speech-language pathology services

Plan of care is goal directed & adjusted based on patient progress/needs

Finding resources for prosthetic/adaptive devices

Clearly communicates with physician regarding :

Plan of Care development and changes (HCFA 485)

Observations & Clinical Changes

Obtaining verbal orders

Completes the following medical record documentation thoroughly, timely and accurately :

Speech-Language Pathology/Therapy Assessment

OASIS Documentation

Progress Notes (as needed)

Discharge/Transfer Record

Communicates appropriately with the other members of healthcare team including :

Nurse

Physical Therapist

Occupational Therapist

Medical Social Worker

Home Health Aide
